Parish Registration

Thanks for taking a moment to register your household's information with our parish's office. You could come into the rectory to register in person, or you may download an online registration form below.  Please fill out the form as completely as possible and then print it out and drop into the collection basket at Church or drop it off at the Rectory.    You are not fully registered at St. John Cantius until you download and fill out the Full Parish Registration Form by clicking the link below
