Parish Registration

Thanks for taking a moment to register your household's information with our parish's office. If you fill out your information below we will contact you as soon as possible so that you could come into the rectory to register in person or you may download an online registration form below.  Please fill out the form as completely as possible and then print it out and drop into the collection basket at Church or drop it off at the Rectory..    You are not fully registered at St. John Cantius until you fill out the Full Parish Registration Form.
